What is Cosopt PF?
Cosopt PF is a carbonic anhydrase inhibitor with a beta-adrenergic receptor blocking agent that is preservative free.
How Does Cosopt Work?
Cosopt is comprised of two drugs: dorzolamide hydrochloride and timolol maleate. Both decrease aqueous production inside the eye.
Dorzolamide hydrochloride is an inhibitor of human carbonic anhydrase II in the ciliary processes inside the eye which are responsible for secreting the aqueous humor.
Timolol maleate is a non-selective beta blocker which also reduces the production of aqueous humor.
Reasons for Using Cosopt PF
Cosopt PF is indicated for the reduction of the intraocular pressure in patients with open-angle glaucoma, ocular hypertension, or secondary glaucoma.
Contraindications for Cosopt PF
Asthma and COPD
Sinus bradycardia, AV block, cardiac failure, cardiogenic shock
Hypersensitivity to any component of Cosopt PF
Precautions for Cosopt PF
Sulfonamide hypersensitivity as dorzolamide is a sulfonamide
Increased reactivity to allergens and less responsive to epinephrine used to treat anaphylactic reactions
Potential of muscle weakness due to the beta blocker
Masking of hypoglycemic symptoms in patients with diabetes mellitus
Masking of thyrotoxicosis
Renal and hepatic impairment
Impairment of beta-adrenergically mediated reflexes during surgery
Corneal endothelial decrease
Precautions in Specific Populations for Cosopt PF
Cosopt PF has not been studied in pregnant and nursing women. It should only be used if the potential risks out-weigh the benefits.
Cosopt PF has been established in children age two and older.
Side Effects for Cosopt PF
Taste perversion or bitter 30%
Conjunctival hyperemia, blurred vision, superficial keratitis, or itching 5-15%
Side effects related to beta blockers carbonic anhydrase inhibitors in general
Administration and Strength of Cosopt PF
Cosopt PF is instilled into the affected eye or eyes two times a day. Dorzolamide 20 mg/mL 2% and Timolol 5 mg/mL 0.5% are the strengths of each of the drugs in Cosopt PF.
How is Cosopt PF Supplied?
Cosopt PF is supplied in a foil pouch containing 15 single-use containers. Discard any unused containers 15 days after first opening the pouch. Cosopt PF is a sterile solution that does not contain a preservative. The solution from one individual unit is to be used immediately after opening and after use any remaining solution should be discarded. For patients who need this type of medication and are allergic to preservatives this is a way to use the medication without any of the preservatives.
